Can’t make it to your school’s event? You are welcome to attend at another participating location IF TIMES ARE LISTED!
To prepare for your student's vaccine visit:

1. Schedule an appointment

Click on the SCHEDULE tab and choose the correct location and date.

If their school is listed as "DURING THE SCHOOL DAY" the time does not matter-nursing staff will work to make the clinic as least disruptive to the student's day as possible.

2. Complete Forms a minimum of 24 hours prior to your confirmed appointment:
Please log on to your student's health portal to complete and submit the following forms
-
NDC Consent to Treatment, Office Policies, and HIPPA
-
NDC Vaccine Consent

3. If all paperwork is completed correctly-you child will be called down to the nurses office at a time that works in their academic schedule. Parents or guardians do not have to be present for the visit.
Children tend to do well with shots when parents are absent and peers are getting them as well. We have many different distraction techniques that we use to help facilitate as calm and tear free process as possible.

4. Nurses Direct Connect accepts Anthem, United, Aetna, Cigna, Tricare and Medicaid. We do verify your benefits before the appointment, but it is ultimately the guardian's responsibility to assure that vaccines are covered appropriately by your insurance provider. You will be responsible for any balance that is not covered by insurance. If your child is uninsured we do have vaccines available for $20 per vaccine.
5. Vaccines are reported in the CHIRP system and the information should flow directly to your child's record at school and at their primary care provider's office.
If you have any issues submitting any of the above information, please message us on the patient portal or email me at Ashley@nursesdirectconnect.com and we would be happy to help.
