To prepare for your student's physical:
1. Schedule an appointment
Click on the SCHEDULE tab and choose the correct location and date. All students are welcome at ANY clinic-no matter which school you attend
2. Complete Forms a minimum of 24 hours prior to your confirmed appointment:
Please log on to your student's health portal to complete and submit the following forms

NDC Consent to Treatment, Office Policies, and HIPPA

NDC First Visit Intake Form
3. At the event, a paper copy of the IHSAA sports physical form will be provided for you to complete.
If your athlete is under 18 and attending without a parent or guardian, they must arrive with a printed and fully completed form in hand.
Please ensure all required signatures are included, and that the medical history section is filled out as thoroughly as possible.
4. The fee will be charged to your credit card or FSA/HSA card on file at the end of your child's appointment.
5. Kindly give us 48 hours notice for any cancelations. Any cancelations after the 48 hours mark will incur a $20 cancelation fee. No show appointments will be charged.
