Can’t make it to your school’s event? You’re welcome to attend at another participating location!
To prepare for your student's physical:

1. Schedule an appointment

Click on the SCHEDULE tab and choose the correct location and date.
​
2. Complete Forms a minimum of 24 hours prior to your confirmed appointment:
Please log on to your student's health portal to complete and submit the following forms

NDC Consent to Treatment, Office Policies, and HIPPA

NDC First Visit Intake Form​

3. At the event, a paper copy of the CYO or IHSAA sports physical form will be provided for you to complete.
​
If your athlete is under 18 and attending without a parent or guardian, they must arrive with a printed and fully completed form in hand.
Please ensure all required signatures are included, and that the medical history section is filled out as thoroughly as possible.

4. A $35 fee will be charged to your credit card or FSA/HSA card on file at the end of your child's appointment.
